A Clinical Observation on Intertrochanteric Fractures of the Femur


Abstract
From January 1971 to January 1977, 16 cases of the intertrochanteric fracture of the femur experienced in Department of Orthopaedic of Ewha University College of Medicine were analyzed.
Patient age ranged from 7 to 77 years, and the case of injury were mostly falling, especially women, in 7 cases, and associated disease, such as sequel of hypertention 4 cases, tumor condition 3 cases, diabetis mellitus 3 cases.
In classification of the intertrochanteric fracture in the adult 3,4 type were 7 cases according to Boyd and Griffin¢¥s method, and 4 cases of children was using to Delbet¢¥s classification.
Conservative method were performed in 6 cases, such as 3 cases in children fractures, 3 cases in adult.
Open reduction and internal fixation were performed in 10 cases, and metal, for internal fixation were Jewett nail in 8 cases, Smith-Peterson nail and plate in 1 case, round steinmann¢¥s pin in 1 case.
Complication in treated cases were infection of urinary system (1 case), decu-bitus ulcer (1 case), upper ward migration of nail (1 case), delayed union (1 case), and expired for 1 year in 4 cases, such as tumor condition 2 cases, sequel of the hypertention 2 cases.
Average time for union of fracture site, means by X-ray finding, and no pain by full weight bearing was about 12 weeks in cases of internal fixation.
The author experienced to difficult management and selection of treatment method at the initially, especially unstable fracture of intertrochanteric region of the femur, and result of treatment was not satisfactory in mind of tde pat-ients in almost case of unstable fracture type.
